The tragedy is forgotten by many, but the 1991 subway derailment catastrophe under Union Square is back in the headlines after Sunday’s deadly Metro-North accident.

The incident that August day killed five, and led to criminal charges and safety changes.  It was the worst transit accident in New York City before Sunday’s derailment at Spuyten Duyvil.
